Litteraturstudie: Implantatbehandling på pasienter med ektodermal dysplasi

Abstract

Hensikt: Ektodermal dysplasi (ED) er en sjelden tilstand som kan føre til anomalier i strukturer av ektodermalt opphav, derunder tenner. Tilstanden kan også føre til funksjonelle og psykologiske problemer, og redusert livskvalitet. Denne litteraturstudien ønsker å ta for seg oral rehabilitering ved tannimplantater av pasienter med ED, og undersøke suksessraten for implantatbehandling på disse pasientene. Videre vil oppgaven også undersøke hvorvidt implantatbehandlingen kan føre til bedre livskvalitet for pasienter med denne diagnosen. Metode: Et elektronisk søk: «ectodermal dysplasia» AND «dental implants» i databasene PubMed, Web of Science, Google Scholar, og Google ble gjort for å ekstrahere data til artikkelen. I tillegg har man sett på studier som undersøker endring i livskvalitet etter implantatbehandling for pasienter med ED. Resultat: 22 studier om implantatbehandling på pasienter med ED ble inkludert. Totalt 952 implantater ble rapportert installert på 154 pasienter. Alderen til pasientene varierte fra 5-68 år. Av 22 studier rapporterte 15 om behov for tilleggsbehandlinger før implantatbehandling. Både konvensjonelle, zygomaticus- og mini-implantater ble beskrevet. Oppfølging varierte fra 12-300 måneder. De rapporterte suksessratene varierte fra 88,5% til 100%. Studier som tok for seg livskvalitet etter implantatinstallasjon på pasienter med ED viste bedring i faktorer som kan føre til økt livskvalitet. Konklusjon: Implantatbehandling på pasienter med ED er et godt behandlingsalternativ med høy rapportert suksessrate. Man må likevel ta hensyn til kjevenes vekst i behandling av yngre pasienter. Resultatene viser også at pasienter med ED som har fått oral rehabilitering i form av implantater opplever forbedring av faktorer som kan gi økt livskvalitet.

Purpose: Ectodermal dysplasia (ED) is a condition that can lead to abnormalities in structures of ectodermal origin, including teeth. The condition might also lead to functional and psychological problems, as well as reduced quality of life. This literature review sought to address the oral rehabilitation of patients with ED, and to investigate the success rate of implant treatment in these patients. Furthermore, the article will also examine whether implant treatment can lead to an improved quality of life for patients with this diagnosis. Methods: An electronic search in the databases PubMed, Web of Science, Google Scholar, and Google was performed to extract data for the article. Results: 22 studies describing implant treatment in patients with ED were included, where 952 implants were reported installed on 154 patients. The age of the patients varied from 5-68 years. Of the 22 studies, 15 reported the need for additional treatments. Conventional, zygomatic- and mini-implants were all described. Follow-up varied from 12-300 months. The reported success rates ranged from 88.5% to 100%. Studies describing quality of life after implant treatment on ED patients reported improvement in factors that could lead to an improved quality of life. Conclusions: Implant treatment in patients with ED is a good treatment option with a high reported success rate. However, the growth of the jaws must be taken into consideration when treating younger patients. The results also show that patients with ED who have received oral rehabilitation in the form of implants experience improvement in factors that might increase their quality of life.
