Holdbarhetsforsøk av blodgassparametere i venøse hepariniserte fullblodsprøver: Effekten av oppbevaringstid, temperatur og prøvebeholder
Abstract
Background: The stability of venous blood gas samples is poorly documented, due to previous studies focusing on arterial samples. Objective: The studies aim was to examine the stability of the venous blood gas analytes pH, the partial pressure of CO2 (PCO2), actual bicarbonate (cHCO3 -), standard Base Excess (SBE) and lactate. Method: The stability was evaluated by considering changes from baseline after 15, 30, 45 and 60 minutes. The samples were collected in both heparintubes (HPR) and blood gas syringes (BGS) containing lithium heparin. The samples were stored at both room temperature (RT) and on crushed ice. The stability was considered in three participant groups: Healthy subjects, healthy subjects with a respiratory alkalosis achieved through hyperventilation and patients undergoing dialysis with a metabolic acidosis. Results: The lactate concentration increased over time which led to a stability <15 minutes under all conditions. The increase was substantially lower when stored on ice. PCO2 were most stable when stored in BGS, but only BGS in RT was stable within the measured time intervals (15 minutes). pH showed a stability of <15 minutes in HPR and of 15 minutes in BGS, independent of temperature. cHCO3 - and SBE were most stable in BGS on ice, with a stability of 60 minutes, because of a small total variation in pH and PCO2. Conclusion: Venous blood gases where most stable when stored inn BGS on ice, except for PCO2. The stability was: pH: 15 minutes, cHCO3 - and SBE: 60 minutes, PCO2 and lactate: < 15 minutes. Bakgrunn: Holdbarheten til venøse blodgassprøver er dårlig dokumentert, og tidligere forskning har fokusert på arterielle prøver. Hensikt: Studien ble utført for å kartlegge holdbarheten til de venøse blodgassparameterne pH, patialtrykket av CO2 (PCO2), aktuell bikarbonat (cHCO3 -), standard Base Excess (SBE) og laktat. Metode: Holdbarheten ble vurdert ved å betrakte endringer i blodgassverdiene fra nullprøvene etter 15, 30, 45 og 60 minutter. Prøvene ble oppbevart i heparinrør (HPR) og blodgasssprøyter (BGS) tilsatt litium heparin, og lagret i romtemperatur (RT) og på knust is. Holdbarheten ble vurdert hos friske givere (normale blodgassverdier), friske givere med respiratorisk alkalose oppnådd gjennom hyperventilering og hos dialysepasienter med metabolsk acidose. Klinisk signifikante endringer ble vurdert ut i fra krav til tillatt bias og totalfeil basert på biologisk variasjon. Resultat: Laktatkonsentrasjonen økte over tid og hadde en holdbarhet på <15 minutter ved alle betingelsene. Økningen var betraktelig lavere ved oppbevaring på is. PCO2 var mest stabil på BGS, men det var kun BGS i RT som viste en holdbarhet innenfor målepunktene (15 minutter). pH hadde en holdbarhet på <15 minutter i HPR og på 15 minutter i BGS, uavhengig av temperatur. cHCO3 - og SBE hadde lengst holdbarhet i BGS på is med en holdbarhet på 60 minutter, på grunn av at det var totalt liten variasjon i pH og PCO2. Konklusjon: Holdbarheten for venøs blodgass var best ved oppbevaring i BGS på is, med unntak av PCO2. Holdbarheten var: pH 15 minutter, cHCO3 - og SBE 60 minutter, PCO2 og laktat < 15 minutter.
