Behandlerrollen og rollen som forskningsmedarbeider i en Randomisert Kontrollert Studie – doble roller og bindinger En kvalitativ fokusgruppeintervju studie

Abstract

Innledning Denne studien belyser behandleres rolle som kliniker og forskningsmedarbeider og deres erfaringer med doble roller og bindinger i en randomisert kontrollert studie (RKS). Metode En kvalitativ fokusgruppeintervjustudie av behandlere som utøvde behandlingen i forskningsprosjektet Funksjon, Aktivitet og Arbeid (FAktA) ble transkribert og analysert ved Systematisk tekstkondensering. Funn Funnene viser at relasjonen og interaksjonen mellom behandler og pasient blir endret i en RKS og påvirket av den, og at bindinger til protokoll og det aktuelle forskningsdesignet virket inn på de valgene som behandlerne tok sammen med pasienten. Behandlingsrelasjonen ble påvirket i begge behandlingsarmene. Behandlerne som underviste i smertemestring kombinert med fysioterapi opplevde det som krevende å ikke kunne tilpasse pasientundervisningen til den enkelte pasient. Retningslinje og bindinger til protokoll blir førende for konsultasjonen også der behandlerne får beskjed om å behandle pasienten som vanlig, slik som innen den psykomotoriske behandlingsgruppen. Behandlerne opplevde ikke pasienten som en vanlig pasient. Konklusjon Behandlingsprotokollen i en RKS bidrar til å endre vilkår som behandling i klinikken vanligvis hviler på. Det var behandlernes erfaring at de mellommenneskelige prosessene i konsultasjonsrommet ble affisert. Behandler følte at forskningsprosjektet ga prioriteringsdilemma, og nødvendiggjorde en krevende balanse i konsultasjonen mellom pasientbehov og bestemmelser i forskningsprotokollen. Utforming av forskningsprotokoll kan ha konsekvenser for pasientbehandling, behandlingsrelasjon og i neste omgang også pasienttilfredshet. Informantene opplevde at pasientene deltok for egen helsegevinst, og jeg har diskutert dette i relasjon til begrepet ”den terapeutiske misforståelsen”.

Introduction This study elucidates the therapist`s roles as clinician and research employee and experience with double roles and bindings in a randomized controlled trial (RCT). Method A qualitative focus group interview study of therapists who performed the treatment in the Function, Activity and Work (FAktA) research project was transcribed and analyzed by using Systematic Text Condensation. Findings The interaction between the therapist and the patient was changed when participating in an RCT. The binding to the research protocol influenced the choices that the therapists took with the patient. The treatment relationship was affected in both treatment arms. The therapists who taught pain management combined with physiotherapy had experienced not being able to adapt the patient education to the individual patient. Guidelines and bindings to the protocol become decisive for the consultation also when the practitioner was instructed to treat the patient as usual, as in the psychomotor treatment group. The therapists experienced that the patients were not regular patients. Conclusion A research protocol will change the interpersonal processes in the consultation room. The informants of this study stated a dilemma between patient handling in research and in clinical work, which necessitated a demanding balance between patient needs and demands of the research protocol. I discuss consequences for the design of research projects. The research protocol will have consequences for patient treatment, treatment relationship and patient satisfaction. The informants experienced that the patient participated for their own health gain, which I discuss in relation to the concept of the therapeutic misconception.
