Oral helse hos barn og unge med autismespekterdiagnose – en vurdering av tannhelsetilbudet

Abstract

Bakgrunn: En autismespekterdiagnose (ASD) er karakterisert ved nedsatt evne til språk og kommunikasjon, et rigid og repetitivt tanke- og atferdsmønster, samt problemer knyttet til sosialt samspill og relasjoner. Utfordringer knyttet til diagnosen kan medføre vansker i tannbehandlingssituasjonen og kan påvirke muligheten til å motta tannhelsetjenester. God oral helse er en viktig del av den generelle helsen. Formål: Studien undersøkte tannhelseatferd og oral helse hos barn og unge med autismespekterdiagnose og hvordan foreldre opplever at den offentlige tannhelsetjenesten muliggjør tannhelseundersøkelser og tannbehandling. Videre ble det studert hvilke faktorer som påvirker foreldrenes grad av fornøydhet med den offentlige tannhelsetjenesten. Metode: Deltakerne i studien inkluderte 327 foreldre til barn og unge i alderen 3-18 år med ASD fra hele Norge. Foreldrene gjennomførte et spørreskjema med 35 spørsmål for å innhente informasjon om oral helse for denne pasientgruppen. Sammenheng mellom variabler ble undersøkt ved kjikvadrattest, samt korrelasjon - og regresjonsanalyser. Resultater: Analysene viste at ulike former for sensorisk hypersensitivitet kan påvirke tannhelsen i negativ retning, samt at tannhelsetjenesten i det alt vesentligste ikke tilrettelegger for denne pasientgruppen i tilstrekkelig grad. At barnet mestret undersøkelse, at tannhelsepersonell tilrettela undersøkelse/behandling etter barnets behov og opplevelse av at tannhelsepersonell hadde kunnskap om ASD var signifikante prediktorer for grad av fornøydhet med den offentlige tannhelsetjenesten. Konklusjon: Tannleger og tannpleiere i den offentlige tannhelsetjenesten har behov for mer kunnskap om barn og unge med ASD for å kunne individuelt tilrettelegge for mestring i tannbehandlingssituasjonen.

Background: Autism spectrum disorder (ASD) is characterized by impaired ability for language and communication, a rigid and repetitive pattern of thoughts and behavior, as well as challenges related to social interaction. Problems related to these diagnostic criteria may lead to difficulties regarding dental treatment, including a reduced possibility of receiving dental care. Good oral health is an important part of the general health. Purpose: This study investigated how parents of children and adolescents with ASD reported that the public dental service is enabling dental examinations and treatment, as well as what affects the parents' satisfaction with the public dental service. Method: Participants in the study included 327 parents of children and adolescents aged 3-18 with ASD from all over Norway. The parents completed a 35-question questionnaire to obtain oral health information for this group of patients. The relationship between variables was investigated by a chi-square test, as well as correlation and regression analysis. Results: The analysis showed that various forms of sensory hypersensitivity can have adverse effect on dental health, and that the dental service do not adequately facilitate this patient group. Significant predictors of parental satisfaction with the public dental service was the child’s mastery, dental professionals’ ability to facilitate the examination/treatment according to the child's needs and the experience of dental professionals’ knowledge of ASD. Conclusion: Dentists and dental hygienists in the public dental service need more knowledge about children and adolescents with ASD in order to individually facilitate mastery in the dental treatment situation.
