Sykepleieres erfaringer med utskrivelse av eldre pasienter til kommunehelsetjenesten og pasientenes medvirkning i denne prosessen - En studie etter iverksettelsen av samhandlingsreformen

Abstract

This thesis has two sections. First there is an introduction, which includes background, method, theory and a discussion. The discussion is meant to highlight the context of the findings from this study. In the second part of the thesis is a draft for an article written after guidelines from Scandinavian Journal of Caring Sciences (SJCS) (attachment 6). Aim: The aim of this study was to describe how nurses working in hospitals experience elderly patients participation when being discharged to a municipal health care service. Nurses’ experience regarding this topic after the introduction of the coordination reform (CRF) was especially emphasized. Design: A qualitative design using focus groups were chosen. Method: Two focus groups with a total of eight nurses participating where conducted. To analyse and interpret the data a framework made by Knodel was used. Findings: From the interviews four topics emerged: Conditional patient participation, stuck between a rock and a hard place, with the subtopics: Stuck between patients and relatives and stuck between ambitions and opportunity. The last two were called: Better documentation and communication –less responsibility and the municipality has a key role. Conclusion: Even though the nurses in this study wanted to let patients participate, this was not always easy. Lack of resources, time limitations and standardized guidelines were factors that made participation demanding. The nurses expressed a positive impression of the CRF, but they also realized that the guidelines it brought with it could prevent patent participation.

Dette er en todelt oppgave, der introduksjonen tar for seg bakgrunn, metode, teori og en begrenset del av drøftingen. Drøftingen i introduksjonen er tatt med for å få en større forståelse av konteksten til studiens tematikk. Den andre delen av oppgaven er et utkast til en artikkel. Artikkelen er skrevet etter retningslinjene til Scandinavian Journal of Caring Sciences (SJCS). Hensikt: Undersøke sykepleieres erfaringer med eldre pasienters medvirkning i utskrivelsen fra sykehus til kommunehelsetjeneste. Studien er utført i lys av samhandlingsreformen (SHR). Design: Studien har en kvalitativ tilnærming og fokusgruppeintervju ble utført. Metode: To fokusgruppeintervju ble foretatt, med til sammen åtte deltakere. Knodel ble brukt som ramme for analysen. Funn: Analysen resulterte i fire tema. Disse er: Betinget brukermedvirkning, sykepleierne mellom barken og veden som har to undertemaer: Skvis mellom pårørende og pasienten og skvis mellom muligheter og ønsker. De to siste temaene er: Bedre dokumentasjon og kommunikasjon, mindre ansvar og kommunen - en nøkkelrolle. Konklusjon: Sykepleierne i denne studien ønsket å la pasienter få medvirke i avgjørelser angående deres utreise, men i praksis kunne dette være utfordrende. Ressursmangler, tidspress og standardisering var faktorer som gjorde dette vanskelig. Sykepleierne hadde en positiv oppfatning av SHR, men poengterte samtidig at noen av retningslinjene som fulgte med den, kunne hindre medvirkning.
