dc.description.abstract | Background: Referred pain due to reduced mobility in the thoracic spine is common, and this pain can often be confused with pain from viscera, specially the heart. There are indications that manual therapy and exercises may have effect on these symptoms. My intention is to find out whether a small clinical study can support these indications. Hypothesis: Does manipulation and mobility training reduce pain and increase function with a patient having hypomobility in the thoracic spine? Method: Single subject experimental design was used to confirm the hypothesis, and one patient was included in the study. The intervention was manipulation and guided mobility training. There were four treatments over a period of two weeks. The outcome measurements were pain and stiffness measured with Numeric Rating Scale, function was measured with lateralflexon and thoraxexpansion and Patientspecific Functionalscale and fear for movement was measured using Fear Avoidence Beliefs Questionnaire. All the tests were performed at baseline and during the first follow-up examination, while pain and stiffness was measured additionally three times in the period of intervention and three and eight weeks post intervention. Results: All the outcome measurements showed clinically significant improvements with the patient in this study. Pain and stiffness decreased by 2,7 and 7 points respectively, and lateral flexion and thoracic expansion showed a positiv trend. The self-selected functions were easier to perform. The patient had low score - under cut-off- on fear avoidance and hence no change on this measure. Conclusion: The study showed a tendency that manipulation and mobility training may have a positive effect on patients with hypomobility in the thoracic spine. Further research through randomized controlled trials with a higher number of subjects is needed to investigate appropriate intervention on this type of musculoskeletal disorder. | en_US |
dc.description.abstract | Bakgrunn: Refererte smerter på grunn av nedsatt bevegelighet i thorax er vanlig, og smertene kan ofte forveksles med smerte fra organer, spesielt fra hjertet. Det er indikasjoner for at manuellterapi og øvelser kan ha effekt på plagene. Jeg ønsker å finne ut om en liten klinisk studie vil kunne støtte opp om disse funnene. Problemstilling: Kan manipulasjon og bevegelighetstrening redusere smerte og øke funksjon hos en pasient med hypomobilitet i thoracalcolumna. Metode: Single subject experimentel design ble benyttet for å få svar på problemstillingen, og en pasient ble inkludert i studien. Intervensjonen besto av manipulasjon og veiledet bevegelighetstrening. Det ble gitt fire behandlinger over to uker. Utfallsmålene var smerte og stivhet målt med Numeric Rating Scale, funksjon målt med lateralfleksjon og thoraxekspansjon samt Pasientspesifikk funksjonsskala og frykt for bevegelse målt med Fear Avoidence Beliefs Questionnaire. Alle testene ble utført ved baseline og første oppfølgingsundersøkelse, mens smerte og stivhet i tillegg ble målt tre ganger i intervensjonsperioden og tre og åtte uker etter intervensjonen. Resultat: Alle utfallsmål viste klinisk viktig bedring hos pasienten i denne studien. Smerte og stivhet falt henholdsvis 2,7 og 7 poeng, og lateralfleksjon og thoraxekspansjon viste en positiv trend. De selvvalgte funksjonene ble lettere å utføre. Forsøkspersonen hadde lave skåre -under cutt-off på fear avoidence og hadde derfor ingen endring. Konklusjon: Studien viste en tendens til at manipulasjon og bevegelighetstrening kan ha effekt på pasienter med hypomobilitet i thoracalcolumna.
